Covered Procedures

CosmetAssure protection extends to the 17 procedures listed below. All patients having one or more of the Covered Procedures must be enrolled in the CosmetAssure program through a participating surgeon to be eligible for coverage.

Rhinoplasty
Thigh Lift
Upper Arm Lift

CosmetAssure enrollment is required on all patients having one or more of these Covered Procedures in the case that (1) regular health insurance does not cover the initial aesthetic procedure(s): and/or (2) patient requires general anesthesia or IV sedation.

This is only a brief description of the coverage(s) available under policy series S30418NUFIC. The Policy will contain reductions, limitations, exclusions and termination provisions. Full details of coverage are contained in the Policy. If there is any conflict between the contents of this document and the Policy, the Policy will govern in all cases.
